
About the Role
An employee in this position may be called upon to fulfill any or all of the following role responsibilities. The description below does not identify all of the functions in which the employee may be required to perform.
-Maintain a Person-Centered Approach in all coordination services.
-Facilitate completion of an assessment plan that recognizes the member/client’s preferences, needs, and
any risk factors that impact their experience of a home or community-based setting.
-Participate in identifying supports and services needed to ensure the member/client’s needs, preferences,
and risk factors are addressed.
-Support client/member with securing non-Medicaid according to the assessment plan, as needed.
-Confirm a transition recommendation from the Transition Options Team.
-Facilitate completion of a transition plan according to member/client’s preference.
-Support member/client’s post-monitoring needs, making adjustments to the assessment plan accordingly.
-Assist members with obtaining necessary documentation for housing voucher application, supporting the
submission of the application, and attend housing orientation with members who have been awarded the
housing voucher.
-Maintain communication with member’s Housing Navigator, Case Manager, and any other authorized
agency/persons, as needed.
-Maintain member’s electronic and hardcopy documentation according to Colorado Department of Health
Care Policy and Financing.
-All responsibilities are time-sensitive, all documentation and services must be completed accordingly.
-Collaborate, as needed, with housing navigation services to obtain a voucher and locate housing.
-Physical presence is required at a member's facility’s discharge to ensure all necessary documentation,
medication (etc.) have been received. Also meet members at the new home to ensure necessary
components and services are in place.
-Other responsibilities as the department needs.

Requirements
Qualifications
Education: Bachelor’s degree in a human behavioral science or related field of study.
Experience: Minimum of one (1) year preferred.
Equivalency: Experience working with long-term service supports (LTSS) population, in a private or public agency or lived experience will substitute for the required education on a year- for-year basis; or
Combination of LTSS experience and education, demonstrating a strong emphasis in a human behavioral science field.
