
About the Role
An employee in this position may be called upon to fulfill any or all of the following role responsibilities. The description below does not identify all of the functions in which the employee may be required to perform.
-Maintain a Person-Centered Approach in all Life Skill Training services.
-Life Skills Training (LST) trainers directly support the client by designing with the client an individualized ----LST program service plan, and by implementing the plan through training with the client to develop and
maintain his/her ability to independently sustain himself/herself physically, emotionally, socially, and
economically in the community.
-Ability to identify supports and services needed to ensure the member’s LST program service plan is
successfully implemented.
-Flexibility in supporting members in varying environments, according to LST program service plan.
-Maintain member’s electronic and hardcopy documentation according to Colorado Department of Health
Care Policy and Financing.
-All time-sensitive responsibilities-including but limited to required trainings, documentation, and direct
member services- must be completed according to the tasks’ time restrictions.
-Collaborate, as needed, with other agencies as needed to support member’s needs.
-Uphold the responsibilities of a mandated reporter.
Requirements
Qualifications
Education: Associate’s degree in a social service or human relations field and experience; Bachelor’s degree in a social service or human relations field and experience.
Experience: Minimum of two (2) years working with individuals with disabilities required with Associates degree; minimum of one (1) year experience working with individuals with disabilities required with a Bachelor's degree.
Equivalency: A licensed healthcare professional with experience in providing functionally based assessments and skills training for individuals with disabilities;
Currently enrolled in a degree program directly related to special education, occupational therapy, therapeutic recreation, and/or teaching with at least three (3) years of experience providing services similar to LST services;
Four (4) years direct care experience teaching or working with needs of individuals with disabilities; or
Four (4) years of lived experience transferable to training designed and directed with the member to develop and maintain his/her ability to sustain himself/herself physically, emotionally, socially and economically in the community; and the provider must ensure that this individual receives member-specific training sufficient to enable the individual to competently provide LST to the member consistent with the LST Plan and the overall Service Plan.
Combination of LTSS experience and education, demonstrating a strong emphasis in a human behavioral science field.
